An Incident Reporting & Resolution App is designed to help organizations efficiently report, track, and resolve incidents. These apps are used in many industries to manage incidents ranging from safety hazards to service disruptions. An effective incident reporting system helps organizations adhere to regulatory standards, prevent future incidents, and foster a culture of safety and accountability.
Incident reporting in the healthcare & welfare sector is crucial for maintaining compliance with legislation and industry certifications, and ensuring the safety of staff, clients, and the public.
In a social work setting, a caseworker reports a safety incident where a client exhibited aggressive behaviour during a home visit. Using a digital reporting app, the caseworker logs the incident immediately, including photos or video evidence captured through the app’s camera feature. This allows the management team to review not only the report but also visual evidence in real time. Incident reviews can be followed up with staff training, updates to protocols and procedures, and re-rating risk assessment. An Incident Management app allows organisations to rapidly respond to incidents and provide written reports to the board, government, and funding bodies. PowerBI provides data and insights in a format that can assist business planning and decision making.
This App enables reporting, tracking, and management of issues such as safety breaches, accidents, or service disruptions. We define six categories of incident. Those that relate to People, or Assets. Incidents that impact on Business Operations. Breaches of protocol and Non-compliance. Incidents that impact on reputation. And finally environmental incidents.
The severity of each incident is rated from 0-5, where zero indicates a near miss, and five represents a serious incident.
The App is responsive and works on any device. Using a smartphone, the frontline staff can report the incident details and take photos. The app then sends notifications, assigning responsibilities, and monitoring progress toward resolution.
Managers can review the incident on a laptop or PC. The app provides two levels of review, typically completed by a supervisor, and then closed out by senior executive. Each step of reporting, investigation and resolution is tracked using the app.
At this point power automate workflow will create a Word or PDF document of all incident details, review and resolution, as entered into the app.
After the incident is closed, it is archived in the app. If required at a later time, all details of the incident can be viewed in a read only format.
Power BI, can be used to gain insights into incidents, by visualizing their frequency and severity, while highlighting trends over time.
By incorporating these features, an Incident Reporting & Resolution App can significantly enhance an organization’s ability to manage incidents effectively, reduce risks, and improve overall safety and operational efficiency.
